  3. Your Telphone & Fax number (if available).
  4. Your current email address (if available).
  5. The type of account you wish to apply for.
  6. Please include Credit Card number, expiry date, and, billing address if paying by credit card.
  7. If you are paying by check, please include a check for the amount of first months charge plus any applicable setup charges.
